HOLD HARMLESS AGREEMENT/PERMISSION SLIP

Parent and student MUST sign and return the Hold Harmless Agreement/Permission Slip with the Medical Emergency Information and Registration forms and payment.  Registration is not confirmed and graduate will not be able to board the bus unless all documents are completed and turned in.

As a parent of _________________________________, I give my graduating son/daughter permission to attend Grad Night 2014 on Friday, May 30, 2014. I understand that this event has been carefully planned by senior parents and will be chaperoned by SSFHS  parents and/or family members. I understand that my registered graduate must be at SSF High School by 11:00pm on May 30, 2014.  I also understand that I will be responsible for picking up my graduate at SSFHS by 12Noon on Sunday, June 1, 2014.

Both my graduating senior and I understand that appropriate behavior is expected at all times and that this is a drug and alcohol free event. The Grad Night 2014 Committee or its representatives will expel any graduate who is found with, using, or under the influence of drugs or alcohol at any time during the night’s activities. Local police will be notified. Parents will be called to pick up their son/daughter for violating this trust, regardless of the hour.

THIS CONSENT HOLDS HARMLESS AND RELEASES MEMBERS OF THE SOUTH SAN FRANCISCO HIGH SCHOOL GRAD NIGHT 2014 COMMITTEE, CALIFORNIA STATE PTA, INCLUDING ALL UNIT, COUNCIL AND DISTRICT PTAS AND ALL OF THEIR OFFICERS, DIRECTORS, MEMBERS AND VOLUNTEERS, THE CHAPERONES, THE SOUTH SAN FRANCISCO UNIFIED HIGH SCHOOL DISTRICT, ALL SSFUSD EMPLOYEES  FROM RESPONSIBILITY FOR INJURIES OR HARM CAUSED TO ANY PARTICIPATING GRADUATE.
