GRAD NIGHT 2014 REGISTRATION FORM

REGISTRATION FORM

This registration form, along with other mandatory forms listed below, is required for every graduate who participates in Grad Night 2014. Reservations are contingent upon being eligible for graduation and participation in graduation events. The form also includes the optional contribution selection.   

IMPORTANT DATES AND FEES
Registration - Now through January 7,2014:    $250.00

Please mail or deliver to the school office all forms and registration payment (including any optional donation)  to:
South San Francisco High School, ATTN: SSFHS Grad Night 2014
400 B Street, South San Francisco, CA 94080

MANDATORY FORMS:  Registration Form, Hold Harmless Agreement/Permission Slip, and Medical Emergency Information Form.

PAYMENT:  Please make checks payable to SSFHS Grad Night 2014.  There are no refunds.
