GRAD NIGHT REGISTRATION FORM

REGISTRATION FORM
This registration form, along with other mandatory forms listed below, is required for every graduate who participates in Grad Night 2014. Reservations are contingent upon being eligible for graduation and participation in graduation events. The form also includes the optional contribution selection.
IMPORTANT DATES AND FEES
Registration - Now through January 7 2014: $250.00
Last date to register: Tuesday, January 7, 2014
Please mail or deliver to the school office all forms and registration payment (including any optional donation) to:

MANDATORY FORMS: Registration Form, Hold Harmless Agreement/Permission Slip,Installments/deposits acknowledgement form and Medical Emergency Information Form. No one will board bus without all Mandatory forms completely filled out and turned in with your full deposit.

PAYMENT: Please make checks payable to SSF PTSA Grad Night with note Grad Night 2014. There are no refunds. No exceptions. All installment payments/deposits arranged are nonrefundable/nontransferrable. Students who fails to complete their installment payments or who decide not to attend after completion of their installment payments/deposit are Nonrefundable/Nontransferrable, those funds will go towards the General Grad Night fund of class of 2014.
Additional note: We are doing fundraisers (general fundraisers for all students:E-waste,Dinner Auction fundraiser, Dec Gift wrap, DVD GradNight/Graduation Ceremony, Tshirts fundraiser etc..)
We will know by next year in May how much we will reimburse partial or full credit to
those students who volunteer their time in the car washes and from all our fundraiser's Revenues.
